Person Centered Supports We are a team spanning from the  Hunter Valley including Scone and Muswellbrook through to Sydney and parts of Queensland.  Our Knowledge of the local area and providers ensure you will have access to a large Variety of supports. we also work remotly with many clients across australia.

 Affinity Support Coordination has a team of people with extensive disability services training and life experience. We offer co-ordination services that meet the needs and goals of NDIS customers requiring childhood services through to youth and adults who may have a wide range of psychiatric, cognitive, physical, sensory, acquired, neurological and/or psychosocial disability and complexity in their lives.

Our Support Coordination service connects NDIS participants with informal, mainstream and specialist services.

We work with you to build independence, improve quality of life and support you overcome any barriers that you may encounter. 

Every client we work with is an individual and deserves supports Tailored to them!
